Referrals

Professionals wishing to make a referral to Project Turnabout can contact our Admissions Department 24-hours a day, 7 days a week at 1-800-862-1453. Our Licensed Admissions Counselors are trained to answer your questions about the programs we offer and admission criteria. Your clinical impressions are valuable to us in the referral process. Any and all information you can provide us will be helpful in planning services and or securing funding.

Examples of information are as follows:
1) Personal Information (name, age, address, employment, gender, etc.)
2) Substance Use History
3) Brief Medical and Psychiatric History Including Names of Current Medications (if available)
4) Name of Family Contact Person
5) Name of Insurance Company, Phone Number, Subscriber Information and Employer* (if available).

*Note: As your are aware, this information is often needed by our financial services department to determine insurance benefits for you.

During the admissions process our Admissions Counselor may request to speak with the potential patient and/or family member. Of course we would want to hear your counsel on this. This conversation is to gather additional information, if needed, and answer any questions they may have about entering treatment at Project Turnabout.

We will do our best to meet your needs for an admission date, though there are times we can get backed up. Please let us know any special considerations or issues you have related to time of admission. Our goal is provide a date, pending sufficient information, to determine the patient meets our criteria for admission (as outlined in state guidelines).

Please know that it is our policy to attempt to secure written permission to contact referents within the first 24 hours of admissions to enable contact with referents. Our goal is to contact the referent upon admission and then again by the assigned counselor throughout the course of treatment.
